What We Do FAST Intensive support services are provided to families with children at risk of out of home placement. This is a short-term (up to 90 days) community-based alternative to psychiatric hospitalization or foster care placement. Intended outcomes are increased safety, stabilization, and ensuring children have a permanent family resource. Wraparound with Intensive Services Wraparound is a consumer driven, community-based alternative to hospitalization and more restrictive care. It is not a set of “services” but a philosophy and team-based process driven by family voice. The process of Wraparound features team-based assessment, planning, action, and evaluation. Crisis Brief crisis resolution for all youth (under 18 years). Clinician meets the youth and family, assesses the immediate risk, and engages the youth and family in a process of crisis resolution and individualized safety planning. An agreed upon action plan is created with the family, and linkages to other helpful supports and services are initiated. Intervention typically runs less than 72 hours. BRS Short-term Therapeutic Foster Care for children who are often struggling in the foster care system. Youth typically struggle with loneliness and mental health issues. CCS works diligently to identify and implement a safe, permanent plan with immediate or extended family. Expanded Wraparound Services Treatment teams provide services to youth and family in the home and community who have needs beyond the needs of families serviced by traditional office-based services. Each of these teams provides 24/7 services and support to families. The youth must be Medicaid eligible and exhibit behaviors for which a mental health diagnosis applies. Youth are typically between the ages of 7 and 17 and there needs to be justification regarding the reasons the youth and family need the more intensive and flexible services of Expanded Wraparound as opposed to traditional in-office therapy and services provided by other community mental health programs. Families may self-refer or may be referred by professionals in the community. Call 253-759-9544 for more information.
